Dehydroepiandrosterone, also known as prasterone, 3-hydroxyandrost-5-en-17-one, dehydroisoandrosterone, prasterone, trans-dehydroandrosterone, or DHEA, is a 17-ketosteroid, which is quantitatively one of the major adrenocortical steroid hormones present in the metabolism of humans. This endogenous androgenic steroid has been shown to have a myriad of biological activities. Dehydroepiandrosterone (DHEA) and its sulfated form, dehydroepiandrosterone-sulfate (DHEA-S) are the principal circulating steroids in humans. These two steroids are synthesized in the adrenal cortex and are normally found at about a 1:1000 molar ratio in serum. Androstenedione is an adrenal steroid hormone. Pregnenolone is a precursor for dehydroepiandrosterone. DHEA is the major secretory product of the human adrenal gland and is the most abundant hormone in the body. It is weakly active as a sex hormone but is a precursor within the body for the androgen testosterone in the male and for estrogenic hormones such as estrone and estradiol in the female. Once DHEA is released into the body from the adrenal gland it is converted into the sulfate ester DHEA-S by the liver. DHEA is a precursor of androstenedione. Androstenedione is a direct precursor of estrone and testosterone in target tissues that possess the appropriate receptors and enzymes. Androstenediols are direct precursors for testosterone after oral administration in adult humans. 19-Norandrostenedione is a precursor for 19-nortestosterone, which has anabolic actions similar to testosterone, with less androgenic actions. 19-Norandrostenedione is a potential precursor for estrone. Testosterone is important for the development and maintenance of male sexual organs and characteristics, behavioral effects, anabolic actions, and metabolic effects for all tissues, especially muscles, liver and kidney. Estrogens are essential for the development and maintenance of female reproductive organs and characteristics, pregnancy, and metabolic effects for all tissues. Although DHEA may act as a precursor for more physiologically active androgenic steroids (e.g. testosterone) it is normally secreted as a hormone in its own right. DHEA is rapidly sulphated to DHEA sulphate (DHEAS) before release into the circulation and it is this latter species which constitutes the main circulating form of the steroid. Outside of the CNS DHEAS is itself inactive. To exert physiological effects it must be desulphated by a steroid sulphate sulphatase at the target site and therefore is able to influence local biological events. DHEA and DHEA-S levels are higher in men, and naturally decline with age in both sexes. Because of this decline, it is suggested that replacing DHEA through supplementation might improve the outcome of diseases and conditions associated with aging. Decreased secretion of DHEA has been associated with advancing age in both sexes, and with certain types of breast cancer in women. Chronic treatment with DHEA in the diet has been shown to increase longevity in rodents by retarding the development of specific diseases associated with particular strains or mutants. Thus, ameliorative biological effects have been established relating to obesity, tumor development, aging, and immune function. DHEA has been proposed for use in treating many medical conditions, such as systemic lupus erythematosus, primary adrenal insufficiency, Addison's disease, reduced libido, obesity, osteoporosis, and fibromyalgia. DHEA can be administered by various routes and is orally active.
